Uh, thanks. I blame the material where I got that sentence, and myself for not seeing at once that the sentence is--as you have described--grammatically and semantically faulty.

Still, should there be the words "years old" after the specified age in your revision?

For those who were already 18 at the time the accident happened, the three years would start from the day they turned 18.

This time, there's no need to add the phrase "years old" to the age. It's already obvious from the sense of the sentence.
